Ongoing adult primary care
Preventive care, evaluation of new concerns and management of common chronic conditions with one physician who knows the larger picture.
Visits and follow-up
Primary care visits and follow-up are included based on clinical need and availability.
Appropriate ways to receive care
Care may take place in person in Poulsbo, by phone or through secure messaging when appropriate.
Weight and metabolic care
Weight-related care may include nutrition and activity guidance, monitoring and medication consideration when clinically appropriate.
Billed separately.
Membership is not health insurance. Members should keep coverage for emergencies, hospital care, specialists, imaging and other care outside the practice.
The monthly fee does not include:
- Emergency or hospital care
- Specialist services
- Imaging and outside diagnostic testing
- Prescription medications and medical devices
- Outside laboratory, DEXA and other body-composition services
Selected routine labs and generic medications may be available at the practice's acquisition cost plus a small supply charge. They are billed separately; availability and pricing vary.

Appointments
Same-day or next-business-day appointments are generally available but not guaranteed. Timing and follow-up depend on clinical need and availability, and some concerns require outside care.
In-person care
Hilmy Health serves adults 18 and older who can travel to the Poulsbo clinic when in-person care is clinically necessary.
Remote follow-up
Established members who reside in Washington may use telehealth when clinically appropriate.
Phone, secure messaging and telehealth are appropriate for non-urgent communication and do not replace an in-person examination when one is clinically necessary.
Medicare eligibility
Hilmy Health is not currently enrolling Medicare beneficiaries. The practice plans to expand eligibility in the future.

Your first two months of membership at no charge.
Available to the first 25 eligible new members or through Oct 31, 2026, whichever comes first. Separately billed services and outside costs are not included. The free 20-minute Meet and Greet is separate from this offer, and membership continues at $125 per month afterward.

Membership is $125 per month for one adult. Primary care visits have no per-visit fee.
Does membership mean I can schedule unlimited visits?
There is no fixed visit limit. Appointments and follow-up depend on clinical need and availability; same-day or next-business-day access is generally available but not guaranteed.
Are laboratory tests and medications included?
No. Selected routine labs and generic medications may be available at acquisition cost plus a small supply charge. Availability and pricing vary.
Is medical weight management included?
Yes. Standard membership includes clinically appropriate evaluation and treatment of weight and metabolic-health concerns. Medications, labs, devices, DEXA scans and other outside services remain separate expenses.
