Direct primary care replaces insurance billing between patient and practice with a predictable monthly membership. Members receive ongoing primary care and follow-up without a separate fee for each visit.
The model supports continuity with one physician who knows your history and follows changes over time.
Membership is not health insurance. Members should keep coverage for emergencies, hospital care, specialists, imaging and other care outside the practice.

Medical Weight Management
Care for overweight, obesity and related metabolic concerns may include individualized nutrition and activity guidance, medication consideration and monitoring. Weight management is included in membership. Medications, labs, monitoring devices, DEXA scans and other outside services are billed separately.

Dr. Mark Hilmy is board-certified in internal medicine and has cared for adults as a hospitalist since 2021. He brings that experience to a smaller practice built for thoughtful decisions and consistent follow-through.

For $125 per month, primary care visits have no per-visit fee. Same-day or next-business-day appointments are generally available but cannot be guaranteed; scheduling and follow-up depend on clinical need and availability.

Established members who reside in Washington may use telehealth when Dr. Hilmy determines it is clinically appropriate. Members must travel to the Poulsbo clinic when an in-person examination or testing is needed; some concerns require local referral.

What is direct primary care?
Direct primary care uses a monthly membership to support an ongoing patient-physician relationship. Primary care visits have no per-visit fee. Membership is not health insurance and does not cover outside care.
Does membership include medical weight management?
Yes. Standard membership includes evaluation and treatment of weight and metabolic-health concerns. Medications, labs, devices, DEXA scans and other outside services remain separate expenses.

Can Medicare beneficiaries join Hilmy Health?
